Notice of Privacy Practices

Each of our practices maintains a HIPAA Notice of Privacy Practices describing how medical information about you may be used and disclosed and how you can get access to it. A copy is available at every office and on request: call the practice you visit, and we will give or mail you a copy at no charge. The full notice will also be posted on this page.

For privacy questions or complaints, contact the practice's Privacy Officer by calling the office. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint.
